Umbilical vs paraumbilical hernia diagram - Satyug Healthcare

Hernia guide • General & laparoscopic surgery

Both hernias appear as a bulge around the belly button, so they are often confused. An umbilical hernia comes out through the navel; a paraumbilical hernia comes out just above or below it. Here is how to tell them apart, what treatment each needs and what surgery costs in India.

Many patients who write to us from Africa, the Middle East and Central Asia say their doctor called the bulge "umbilical" and the scan report called it "paraumbilical" - and they want to know if these are two different diseases. The short answer: they are close cousins. Both are a weak spot in the belly wall around the navel through which fat or a loop of bowel pushes out. The difference is the exact doorway - and that changes who gets it, whether it can close by itself and how urgent repair is.

At Satyug Healthcare we coordinate hernia surgery for international patients at accredited hospitals in India. This guide explains the differences in plain language with pictures, so you can talk to your surgeon with confidence. For full details on each condition, read our guides on paraumbilical hernia and umbilical hernia in adults and children.

● In simple words

Think of the belly wall as a strong curtain with one natural hole - the navel. An umbilical hernia pushes out through that hole. A paraumbilical hernia tears a new small gap right next to it, in the midline band called the linea alba. Babies usually get the first kind; adults, especially women, more often get the second.

❓What Is the Difference Between an Umbilical and a Paraumbilical Hernia?

Umbilical hernia: fat, fluid or bowel pushes through the umbilical ring - the opening in the belly wall that the umbilical cord passed through before birth. In babies the ring has simply not closed yet. In adults it is stretched open again by pressure inside the belly.

Paraumbilical hernia: the bulge comes through a weak spot in the linea alba (the tough midline band between the two "six-pack" muscles) just above or, less often, just below the navel. It is rare in children and most common in adults aged about 35-50, with women affected about five times more often than men.

✎ Why doctors sometimes use both words

The European Hernia Society defines an umbilical hernia as any belly-wall hernia within 3 cm above or below the navel. So many surgeons and guidelines group paraumbilical hernias under "umbilical hernia" - and in adults both are usually treated the same way.

☰Types of Umbilical and Paraumbilical Hernia

1Congenital umbilical (babies)

The navel ring has not closed after birth. Seen in about 15-23% of newborns; most close by themselves.

2Acquired umbilical (adults)

About 90% of adult umbilical hernias develop later in life, when weight, pregnancy or fluid stretch the ring.

3Supra-umbilical (paraumbilical)

Just above the navel - the most common paraumbilical position.

4Infra-umbilical (paraumbilical)

Just below the navel - less common.

Surgeons also describe hernias by size of the gap (European and Americas Hernia Societies): small under 1 cm, medium 1-4 cm and large over 4 cm - and by behaviour:

  • Reducible - the bulge goes back in when you lie down or press gently.
  • Incarcerated (stuck) - the contents cannot be pushed back.
  • Strangulated - the blood supply to the trapped fat or bowel is cut off. This is an emergency.

⚠Symptoms: How Do They Feel?

The symptoms of the two hernias are almost the same. What differs is where the lump sits.

●A soft bulge

At the navel (umbilical) or just above/below it (paraumbilical); bigger when you cough, stand or strain.

●Goes in when lying down

Many hernias get smaller or disappear when you lie flat.

●Ache or pressure

Pain is the most common complaint - reported by about 44% of adults with a symptomatic umbilical hernia.

●In babies

A soft, usually painless lump at the belly button that bulges when the baby cries; from pea-size to about 2 cm.

⚠ Go to hospital immediately if you notice

  • Severe or worsening pain at the lump
  • The lump becomes hard, will not go back in, or the skin turns red, purple or dark
  • Nausea, vomiting or not being able to keep food down
  • You cannot pass wind or open your bowels

⚲How Are They Diagnosed?

1

Examination by a surgeon

Usually enough. The surgeon looks and feels the lump standing and lying down, and while you cough, and measures the gap with the fingertips.

2

Ultrasound

Quick and low-cost; confirms the hernia, its size and contents when the lump is small or unclear. It is less reliable in very large hernias or obesity.

3

CT scan

Shows the exact size of the gap, the contents and any other hernias - useful for large, recurrent or complex hernias and for planning keyhole repair.

4

MRI (if needed)

Used when ultrasound and CT are unclear; about 92% sensitive and 95% specific for belly-wall hernias.

5

Tests before surgery

Blood tests, ECG and a chest X-ray as needed, and an anaesthesia check-up.

⚕Treatment Options

Treatment depends far more on age, size of the gap and symptoms than on the exact name of the hernia.

1Watchful waiting (most babies, some adults)

Most baby umbilical hernias close by themselves, so doctors usually wait until about age 4-5 unless there are problems. Adults with a small hernia that causes no symptoms may also be offered careful watching, because the yearly risk of strangulation in such cases is under 1%. However, about 65% of adults with an umbilical hernia eventually need surgery, and hernias in adults do not heal on their own.

2Open stitch (suture) repair

Through a small curved cut near the navel, the bulge is pushed back and the gap is closed with strong stitches. It suits very small hernias (about 2 cm or less) and is the standard repair for children. In adults, for gaps of 2 cm or more, stitch-only repair has a recurrence rate of about 10-14%.

3Open mesh repair - the guideline choice for most adults

The 2020 European and Americas Hernia Society guideline recommends using mesh for umbilical and epigastric hernia repair to reduce the chance of the hernia coming back, and says most can be repaired by an open approach with a flat mesh placed behind the muscle (preperitoneal). In a randomised trial of 300 adults with 1-4 cm hernias, the hernia came back in 4% after mesh repair vs 12% after stitches alone.

4Laparoscopic (keyhole) mesh repair

Through 3 small cuts away from the navel, the surgeon places a larger mesh from inside the belly. The guideline suggests considering it when the gap is large or the patient has a higher risk of wound problems (for example obesity). A review of 16,549 patients found open repair had more wound infections and recurrences than laparoscopic repair, while keyhole operations took longer.

5Robotic mesh repair

A robot-assisted keyhole repair for complex or recurrent hernias in selected hospitals. It allows precise stitching but usually takes longer and costs more.

6Emergency repair

A stuck (incarcerated) or strangulated hernia needs urgent surgery, sometimes with removal of damaged bowel or fat. Planned repair before this happens is safer.

Decision guide: who needs umbilical or paraumbilical hernia surgery and when
PICTURESimple decision guide - child vs adult, and the emergency signs

✔ DO

✔ Get a surgeon to examine any new lump near the navel

✔ Learn the emergency signs and act fast

✔ Stop smoking at least 4 weeks before surgery and aim for a healthy weight

✔ Ask your surgeon which repair (mesh, open or keyhole) suits your size of hernia

✖ DON'T

✖ Do not tape or strap a baby's navel, or push a hernia in yourself

✖ Do not ignore a lump that becomes hard, painful or discoloured

✖ Do not lift heavy weights while waiting for surgery

✖ Do not assume an adult hernia will go away by itself

♥Prognosis: What Is the Outlook?

✔ Good news

Both hernias are very treatable. After mesh repair the hernia comes back in only about 0-3% of adults, and most people return to desk work within 1-2 weeks, with full recovery in 4-6 weeks. In children, most hernias close by themselves, and after surgery the long-term recurrence risk is about 2%.

Results are poorer when a hernia is repaired as an emergency, when stitches alone are used for gaps over 2 cm, and in people who smoke, have diabetes, liver failure or severe obesity - so treating these before surgery matters.

❓Frequently Asked Questions

Q. Is a paraumbilical hernia the same as an umbilical hernia?

Not exactly. An umbilical hernia comes through the navel ring itself; a paraumbilical hernia comes through the midline just above or below the navel. Because both are within 3 cm of the navel, the European Hernia Society groups them together, and in adults they are repaired in the same way.

Q. Which one is more dangerous?

Paraumbilical hernias often have a narrow neck, which makes trapping of fat or bowel more likely. Baby umbilical hernias rarely cause trouble. Any hernia that becomes hard, painful or discoloured is an emergency.

Q. Can either hernia go away without surgery?

Baby umbilical hernias usually close by age 4-5. Hernias in adults - umbilical or paraumbilical - do not heal on their own and tend to grow, so planned repair is usually advised.

Q. Is mesh always needed?

Not always. Very small gaps (about 2 cm or less) and children are often repaired with stitches. For most adult hernias, guidelines recommend mesh because it lowers the chance of the hernia coming back (4% vs 12% in a randomised trial of 1-4 cm hernias).

Q. How much does the operation cost in India?

The Satyug hernia repair package starts at USD 3,020 for one hernia with a 3-day single-room stay. Large or complex hernias cost about USD 4,000-6,000 and robotic repair about USD 5,500-8,000.
